I rather pick my hotels for convenience and price.
While larger and business-class hotels will usually have a buffet-style breakfast, be aware that very minimal breakfasts are the Italian tradition. You're paying for it; even if they don't quote the price separately, you usually can ask how much with or without breakfast. only one visit so nothing to compare it to, but we stayed at the Duca di York, and I recall they had a great breakfast buffet. But as noted, it was definitely not a budget hotel. In a central location, but a bit hard to find if you haven't located it on a map.
Antica Locanda Leonardo had a wonderful breakfast that stood out above all others on a trip to Italy.
It is also in a quiet location, set back from the street with a courtyard.
I liked the breakfast where I stayed, very much, and would recommend it if it didn't have to be "the best".
I haven't tried all the hotels so I have no idea if it is the best, or even what might be.

For me, I usually go by location and price, as long as it has a decent breakfast. I'm pretty easily pleased, especially in Italy.

I'll go with Nigel's reply. Location and price trumps the breakfast menu every time. We're had great and we've had minimal but it's what we saw and did during the day that mattered.
Agree with location and price, but the very Italian breakfast at the reasonably priced Berna is above average.
Milano is one city where I don't look for "breakfast included", I usually stay at Hotel Garda (breakfast is extra) and have breakfast at VyTa, just inside the train station, then take the metro, bus or tram to my destination.
We love the breakfast buffet at the Hotel Berna. It is also conveniently located and offers reasonable, for Milano, prices. In the evenings the barman lays out an aperitivo buffet that can suffice for a light supper.
I didn't mention it in my first post but because Jerry hasn't come back and it has been mentioned a few times already - I was in fact referring to my very pleasant breakfast at the Berna. Interesting to us is if they put you in one of the lesser priced but very nice rooms across the street in the annex, the same breakfast is included.
The Berna does indeed have a good breakfast 'spread', but they lost points by forgetting my coffee order. Small point I know, but I was in dire need that day.
I'll second Berna but in my case, they didn't forget my coffee.
It's located near the train station which could be good or bad depending on your situation.
