Can you please give a general idea of the party size and what the general interests of your group are first? Physical limitations, if any. Also budget concerns, etc. Thanks.

Bergamo can be visited by train. The medieval center on top of the hill is accessed by funicular.

I have done many day trips from Milan, but not by tour group. I just hop on a train. There are lots of options - among my favorites are Bergamo (1 hour), Lake Como (Varenna - 1 hour), Lake Lugano (1 hour), Pavia (less than 1 hour), Bologna (1 hours), Parma (1.5 hr), Verona (about 1.5 hours). In all these towns the rail station is in the town center and they can all be explored on foot. Read up on them in a guidebook, in most cases you can pick up a map at the TI which is usually near the train station. While some of them 'warrant' more time, all of them can be enjoyed in a day trip.
