Starting with 3 nights in Tivoli and driving to Martinsicuro on the Adriatic (friend’s) for 4 nights. Would like recommendations on smaller towns to visit in Tuscany (not Florence or Milan). Looking for 3 hours or less driving. Possible scenario:
Perugia
Lucca
Portovenere (day trips on boat to Cinque Terre)
Parma
Lake Orta
Flight back to Seattle from Milan Malpensa

My first thought when I look at your suggested itinerary is that I wouldn't need a car for Lucca or Portovenere. And it felt like you were jumping around a little, but when I look at a map it seems doable.
However, when I look at where Martinsicuro is it seems to me that exploring Umbria would be ideal, particularly as you are planning to have a car. Spello, Assissi, Perugia, Orvieto are all closer to you than Lucca. You could also do the Tuscany area with Montepulciano as you mention wine. Food is great in those regions too.
You don't say how much time you have after your first 7 nights. That would make a difference to recommendations too.
