Since most ATM's now use chip readers, skimmers would not be an issue with this technology.
Make sure you use a bank's atm. Those belonging to private companies have nasty fees and exchange rates. Have a great trip.

Exchange rates do not vary by ATM, if it does, you are doing DCC or an exchange, not an ATM transaction. As long as you indicate to do the transaction in euros, rather than your home currency, you are fine. If you are not familiar with DCC, do some searching, it affects credit card use as well as ATM transactions.
As for fees, many bank ATMs, particularly at airports and other tourist areas, charge a fee of a 3-5 euro typically. If you see a fee you don't like, move on, but to get needed cash once (couple hundred euro will last you a week or two. or more) paying a few euro is no sin.
As for skimmers, it is really not an issue in most of Europe. Security measures in place really limit what a thief can do with mag scanned info.
Edit: I will add, ATMs are operated by a number of entities at Malpensa. There are some in Baggage Claim, but I would skip those and head to the Arrivals area, and you should have a couple choices, including some from Posti Italiane (The Italian Postal system) which just maybe do not charge a fee.
