If fashion is your focus do not miss the Armani Silos which is a custom built museum in an old factory. You can wander around the Armani clothes on custom built mannequins and get up close to them not just see them from behind a velvet rope.

But why do you need a tour to go shopping in Milan?
I'd rather spend the money for the tour on buying things.

The RS guidebook has a suggested self-guided walking tour of hgih end window shopping in the Quardrilateral section of Milan, a short walk from the Duomo. I enjoyed spending a little time looking at the high end window displays and showrooms and seeing the well-dressed high end shoppers in the streets. I felt it gave me a little taste of that aspect of Milan but I was not looking to buy anything.
