My wife and I will be spending 4 nights in Tuscany in mid October. What recommendations do you have for either a hotel or B&B preferably in a town with at least a few restaurants?

Tuscany is a fairly large region. It would help if you could provide some indication of towns you might be interested in visiting.
If you want a larger city in Tuscany, Florence is a good choice as it has LOTS of great restaurants, and good choices for Hotels or B&B's.
If you want somewhat of a smaller city, Siena would be a good choice. Again, good choices for lodgings and a fairly good selection of restaurants.
You may find it helpful to browse through the Italy 2010 Guidebook, as you may find a town listed in the book that really piques your curiosity.
