Any recommendations for how to hire a driver to take me from the airport in Milan to Varenna this September?
And any idea what the cost would be?

You can Google “NCC Malpensa”. Lots of companies will pop up.
NCC stands for Noleggio Con Conducente (Car Hire with Driver) and it is the official designation of licensed limo/car drivers in Italy,
The cost should be around 200€. Travel time should be around 90 min, traffic permitting.
If you want to save you can go by train. Take the Malpensa Express from Malpensa to Milano Centrale, then another regional train from Milano Centrale to Varenna. You can be there in less than 2 hours for just 20€ per person.

We contacted our hotel in Varenna and used their recommendation. It was a couple of years ago, 150 euros at that time so it’s probably more. He was waiting for us at arrivals with a sign. Depends on your budget and comfort level coming off international flight, train is much cheaper but you do have to make a change in Milan Centrale and about a ten minute walk from Varenna train station to town square. We’ve done both.
In June we are using Airport Taxi Transfers to pick us up at our hotel in Varenna and drive us to our hotel in Milan. We decided to splurge because I've read many times that the train from Varenna to Milan is very, very crowded, and we would end up standing for the whole ride (1 hour) with our luggage. We are in our 70's and didn't want to do that.
