Lake Garda / Orvieto / Piemonte and ???

I've planned several Italy trips, but am struggling this time around. We have the first three weeks in September to spend in Italy and then we're doing a river cruise from Bordeaux.

We're thinking Lake Garda (we haven't been - where to stay and how much time do we need? Perhaps a aide trip to Ortisei. We've never been to the Domomites)
Orvieto - We .were there once and want to return to Locanda Rosati and spend more time in the area (How many nights do you recommend? We'd like to visit all the surrounding towns.)
Piemonte (we were there several years ago and would like to return (I see that getting from Orvieto to Piemonte isn't easy.)

We'd thought of going back to Puglia, but the travel times via train would essentially take two days out of our itinerary, so it's not feeling quite right.

I'm open to any and all suggestions. Thank you

Suggested minimum number of nights at each location.
Lake Garda: 3 nights (north side more beautiful, because of mountains around)
Ortisei/Val Gardena: 3+ nights (distance from Ortisei to Lake Garda is up to 2 hr drive)
Orvieto: 2 nights should be enough, since you've been there already.
Piemonte is a large region (the largest of the 18 mainland regions of Italy), so the number of necessary nights depend on what you want to see.
Piemonte is not difficult to reach from Orvieto, in spite of the long distance. You basically need to take a train to Florence, then a high speed train to Turin (assuming that is the city in Piedmont you want to go to).
Puglia is on the opposite side of the boot, so it's hard to include both extremities of the peninsula in a 20 day trip. Also it's a very long region, with limited rail options (basically only the coastal line to Lecce is served well).
