We are going to Italy late october, we will be in Rome for 6 days, then head to Positano for 4 days. We then have 4 days open before we fly out of Rome, We need suggestions??
This will be the end of October so I know it will be getting slow.
Arezzo - Florence - Siena - Orvieto ??
4 nights in Florence would give you 2 full days in the city, and another day to head to Siena.
Do you have a guide book for Italy yet? We got one of RS's and it certainly helped in our planning for our stay in Italy. I think you could expect average temperatures of 66 hi/ 52 lo with maybe 21 dry days in late October in Rome if I read my crystal ball correctly. Happy travels.
As Charlie suggested, get a guidebook to help enhance your trip with respect to those 4 days.
The Big 3 in Italy are Rome, Florence and Italy.
Other areas include Tuscany country villages.

How about a day in Pompeii, it was a wonderful experience to see it.
We love Italy in late October - Orvieto is beautiful and much more peaceful than in summer , same for Assisi - other great spot is Padua - even have time for a day trip to Venice.
Last year we were in Italy from mid Ocober to early November - it rained 1 day
