Ideas for Genoa region?

My wife and I have done a Rome/Florence/Venice tour (loved it of course) and would like to get back to Italy and see other areas.

I am considering a trip that includes some ancestry research in the town of Lumarzo (15 miles from Genoa) and am looking for recommendations for the region.

It appears that Milan is the best flight destination (reasonable direct flights from NYC) so I’m looking for recommendations on:

Whether to rent a car to tour the area or rely on rail? (I don’t mind driving, it will be 3 adults).

Must-see destinations in the area? We’d mainly like to experience the local culture/lifestyle/food, not stay in one place, but take a leisurely pace, leave our options open.

Any thoughts or recommendations are welcome!

Decide what you want to see first, then decide whether you want or need to rent a car. I love trains, and Italy has a great train system, but if you pick some towns that are difficult to get to by train, then renting a car makes more sense.

Of course, you can explore Genoa itself.

Many people visit the Italian Riviera, which has beautiful scenery and lovely towns. The Cinque Terre region is very popular and busy with tourists but great for hiking, but there are plenty of other nice towns that aren't so touristy north of there you could visit instead.

Check out the town of Apricale. There are also a couple of smaller towns above it, whose names escape me. Somewhat off the tourist track, but interesting and with the usual Italian complement of good food and wine. Car a must.
