I’m flying from Bolzano to Antwerp at 9am on a Thursday via SkyAlps. The airport appears very small. Do I need to follow the 3-hour rule for international flights? Or can I arrive a bit later? Also, should I trust the bus from Ortisei that morning or book a private transfer?

It's an Intra Europe flight- not really International so 2 hour rule - but I would check with the airline.
I would book a driver.

Check indeed with the airline. With such small airports there is often not much to keep you occupied if you arrive very early, so you often are fine just arriving an hour early.
