No way to predict the wait in security line and nobody skips that, not even a guided tour.
As with any venue being there at opening or waiting til later in day will usually give you the shortest wait
I was on a RS TOUR OF Italy 9 years ago so didn’t have to queue. I always wanted to come back on my own, so here I am. Just wonder if the line can snake around the huge square.

A few years ago, New Years 2019/2020, we stood on a medium length security line. The security lines were off on the right as you face St. Peter's and there were several lines. Rome was mobbed at this time of the year, New Years, and getting into St. Peter's was not difficult. We did not have audio guides and do not recall if they were available. We were clueless about COVID then!
When we went in 2012, when we left the Sistine Chapel we left via the rear door on the right, not the door out to the street. By doing that we didn’t have to go through the Basilica security. We went straight into the basilica. No idea if one can still do that.
That door has been closed for some time. No idea when or if it is going to reopen, according to our guide in November.
a couple of years before covid my wife and I were in a queue diagonally (not around the perimeter) of the square in front. Maybe half way across, maybe a little more.
It looked long but moved quite quickly - maybe 15 minutes or so. No probs.
Rick Steves does a good audio guide and it is free... you download it here and even listen to it through once or twice before going in and everything will be right where you expected it to be.
First thing in the morning, before “vacationers” get up, is the best time. Get there a few minutes before opening time.
Echoing Laurel: get there a little before opening time, and download Rick's guide. The security line moves quickly so don't worry if you do end up kicking heels in it for a bit.
