Early flight back to the States. Looking at Best Western 3 miles from the airport. Not sure about early morning shuttle; not so good reviews. Perhaps a private car?

In 2012 we stayed at B&B Ostia Antica, which is of course a few more miles out from FCO than the Best Western. It is my understanding that the B&B may offer private shuttle service or arrange for a taxi service on your behalf. I can tell you that the proprietors of this fine and reasonably priced B&B offer great customer service. Worth checking into, I'd say.
And...if you do stay at B& B Ostia Antica, and have the time to spare, be sure to ask the staff how to get to Il Monumento in Ostia Antica for dinner. My sister and husband still talk about the Pollo Caciattora that they enjoyed there in 2012.
We stayed at the Best Western in Fiumicino on the way to Istanbul in 2014. I wouldn't do it again, although the adjacent restaurant was quite good.
On the way back, we stayed at the Hotel Tiber down on the water. We loved it, but we didn't care for the restaurant there. We did enjoy eating in other places in town, especially the calamari down the street at GePaGi.
I'm not sure what the problem with the shuttle was. It serves many of the hotels in town and it seemed reasonable at €6 per person. It gets to the airport very quickly, especially if the driver is one that takes the back roads. The schedule posted started very early and had some set pickup times, but we found that special arrangements could be made.
This canned Rome2rio search link provides some other options with approximate times and costs. Please note that the hotel(s) shuttle took more like 15 minutes or less with the back way drivers.
We've booked at the Hilton Garden Inn Fiumicino which is right by the airport and on the shuttle. Haven't tried it yet, but it gets good reviews.

The Hilton Rome Airport is the easiest choice, as it's connected to the airport by a walkway, which leads right into Terminals 1 / 2 / 3 - no shuttle required. For an early morning flight, that's a good option.
Not sure why you want/need to stay near the airport. If you will be in Rome for your last day, why not stay the night in Rome and leave for the airport in the morning? Even if you have an early am flight there are very reliable shuttle services that will get you there on time. After all, you are in Italy so enjoy every last minute.
I have done both and much prefer to leave Rome very early in the am when there is little or no traffic and I look forward to snoozing on the plane. Spending an evening in Fiumicino is the pits.
I am with Tony. We have taken a shared shuttle to the airport as early as 5.30. At that time of day it was. 30 min run.
We too really enjoyed our stay at Hotel Tiber. No problems with the shuttle. Great restaurants nearby and a good, free breakfast served beginning very early (before 6am).
We had arrived at FCO on a late afternoon flight from Palermo so staying by the airport was a no-brainer. It definitely was not the pits, but rather a lovely, stress-free way to end our trip.
Stay the last night in Rome and leave for the airport in the morning. We've taken early am flights from there a number of times and have never had a problem getting to the airport with plenty of time to spare.
Thanks everyone; good, good info to help make a decision. Yes, I get that an extra night in Rome should not be missed, but we will be coming from Cerveteri, already south of Rome. Just wanted to get a bit closer...

We stayed in the Seccy Hotel in Fuimicino the night before our flight. Large rooms,reasonable price, breakfast, the town has many restaurant options that actually seemed more Italian than some of the large town. €20 euro ride to the airport. I would go back here again.
