Chris, you have to register to access the special path for pilgrims to follow to-and-through the door. Visitors not interested in the pilgrimage enter St Peter's through the normal entry. Registration is also necessary to participate in some of the Jubilee Year events. You can find information here:

As info the registration process took us about an hour. All told it took two e-mails with filled out forms and two e-mails back from the Jubilee folks in Rome to complete the process.
We'll be there in late May.

Just an FYI. When we were there over Christmas, there were times when the ushers we only allowing the pilgrims through the door and other times when they were letting anyone go through. If you go the reservation route, you have to start about a mile from St Peters and walk a prescribed route.
