Help with Form for Entry into Puglia

Hi, I am extremely confused about this form that is required for entry into Puglia. Can anyone shed any light on the matter? We are flying into Rome from the US and taking the train down to Bari the following morning. Allegedly, there is some form we must fill out for entry into Puglia but it’s in Italian and when my husband uses his translation app, it still doesn’t make sense. Can someone tell me if we really need this form and who would be collecting it? Our hotels know nothing about it.

I'm doing the same thing you are in two weeks. Just filled this out online. Just google the above and you'll find the form in English. From what I read somewhere, it's a tracking form the government uses to locate you if you were exposed to someone with the virus on your flight. It's only useful for the first few days, so you need only put in your first location.
I'm no expert, but this is what I think it's all about. It probably just goes into some digital file somewhere never to be seen again.

Thank you both for your responses. I am not referring to the passenger locator form. There is something separate that is required for entry specifically into Puglia. That’s where I am having the issue.

Sandra, thanks for your link with the English translations. This looks to me like something that might have been required in pre-vaccine times. Does anyone have more info?
