Are you flying in to Rome Fiumicino airport? Have you decided where you're staying in Tuscany? I will assume you're not staying in Florence - you wouldn't want, or need, a car there.

Unless your goal is to sightsee along the way (doesn't sound like it) I'd suggest taking the train into Tuscany. If you're staying near Florence, you can take the train to Florence S.M.N station and there are car rental agencies very near the station. You can compare prices there with other cities that may be closer to your lodging.

There is a Hertz Rental Car just outside of the train station in Florence.

If there is a hertz rental car just outside the station in Florence it probably belongs to someone else who rented it, because there are no Hertz rental offices outside the Florence station. One has to walk to Borgo Ognissanti, a street a few blocks away (about 10+ min walk).

We need more info on your plans. Where exactly will you be before going to Tuscany, during your Tuscany stay, and after Tuscany?

For now I can tell you that most of us here use a consolidator, such as www.autoeurope.com, to rent cars.

If you are heading straight to Montepulciano from FCO, it will be easy to rent a car at the airport and drive directly. You will not go through Rome. You will travel on a multi-lane highway until you reach the Autostrada. See this map.

That's what we did when we stayed at an agriturismo just outside of Greve. We had a morning arrival and walked over to the Hertz office. Since I'm a Hertz Gold member, we went straight to the parking deck and picked up our car.

The drive was easy. I brought our Garmin. I had purchased the Italy/Greece SD card and before we left, I had saved several of the locations we planned to visit, including our agriturismo. Before we left the parking deck, I set up the Garmin, changing it from mph to kph. I then looked up the saved address of the agriturismo and pressed "Go!". The Garmin guided us to their front door.

Study up on the signage and general instructions regarding driving in Italy. It is really easy if you know what to expect. Just be sure to follow all rules, including strict adherence to the speed limits.

On the day before our return flight, we drove back to FCO and returned the car. We checked into the Hilton and took their free shuttle into Rome. There we had dinner, then took the Walks of Italy Evening Stroll. It was a great finish to a great trip. The shuttle took us back to the Hilton, then a free shuttle took us to our terminal the next morning.

If you don't want to rent in Rome and drive from there- you can take a train Orvieto or Chiusi and pick up rental car there. The only issue is availability- if you need an automatic it is not guaranteed you will get that at a smaller rental location.
I recommend using Auto Europe for your rental. And be sure to get your IDP.

Kemwel offers car rentals to pick up in Chuisi. Auto Europe offers cars for Orvieto.

Where are going to next or flying out of? Dropping car at a different location does not cost much, if anything.

Picking up rental cars at Rome FCO is easy and , as noted above, it is and easy autostrada( interstate type highway) to Tuscany.
Book with AutoEurope.com as they offer good deals and will stand behind you if there are any problems. If you prefer an automatic transmission, there is a better chance of getting one at the airport.

We are picking up a car at Hertz in Chiusi and staying near Montepulciano. I figured it was easy to hop the train from Rome and break into driving in Italy slowly. We drop it back in Chiusi after a week and take the train to Florence. I looked at AutoEurope but we got a better price from the Hertz website.
