Any recommendations for guided tours of the Colosseum and Vatican City outside of the guides and companies I've read in Rick's "Rome" book? I'm looking for the full experience at the Colosseum -- arena floor, second level, third level (attic) and underground, along with Roman Forum and Palatine Hill. Heading to Rome next May.
I've never seen a tour which includes everything at the Colosseum/Forum/Palatine you've listed. In fact, I've yet to see one that includes both the underground and the attic. I can't imagine a guide could even get tickets for adjoining time slots that would cover the full list.
Great point. May have to lower my expectations on the Colosseum guided tour.

Walks of Italy is not highly reviewed by me. They cancelled my tour of the Underground Colosseum 2 days before the tour. I knew that getting tickets was a challenge and that many others had also had their tours canceled at the last minute. I expected I had a 50-50 chance of actually getting to go on the tour. However, it did bother me that even when I contacted them a week before the tour to confirm it was going to happen they cheerfully assured me that it would. A few days later, they cancelled.

Just curious if the underground of the Colosseum is worth it?
The "worth it" question is always subjective, and you'll probably get a variety of opinions. My own is that you can see down into the uncovered parts of it from multiple places above so the pain of trying to land those tickets/tours wouldn't be worth it for me. But you do you. :O)
