Would my GPS work properly in Italy if I downloaded Europe on it.

Yes, they work once you load the EU maps. You can buy SD microcards with EU map on Amazon.

Pardon my cheekiness, but the guarantee above, needless to say it will only work in Italy if it worked properly at home.
If you are planning multiple trips to Italy in the years to come you might consider the DVD version, which can be upgraded to unlimited updates to the maps (also from Amazon). You will still need an SD card for the maps as there will not be enough room on your base unit. Also, there are sites out there that you can download POIs from. Additionally, there are also open source routable maps for free for Europe (but not as detailed as the paid ones). I am using the open source one for Japan.
Edit Add: Used the Garmin Europe a couple of years ago (DVD version) and it worked great: I mainly used it in pedestrian mode in the cities but my only negative is that it did not have the bus systems included, metro and tram info were OK
On the road most of the directions were right on! Good idea to have a good map as backup.
