I am planning to fly from DC to Rome and take a train to Florence. I would appreciate your help to understand the logistics: is there access from the international airport to the train station? If that is the case, is it a long walk? Can you take a taxi or is there a shuttle bus? I want to make the process easy for my wife that has some back problems.

I assume you are asking about Fiumicino Airport and not about Ciampino.
Just follow the yellow signs saying "Train" and in around 20 minutes you'll be at the airport station. Ask for a through ticket to Firenze SM Novella if you are staying in Central Florence.

Get the Trenilaia app, tickets after retrieving your luggage. If not a direct train there is one change downtown, referring to the train number on the departure board to locate the correct platform.

if I can rent a luggage car and leave them at the train station?
Following this post, as we have the exact same scenario in Mid-March.
My understanding is that the train station at Fiumicino is near Terminal 3 and you can get a fast train to almost anywhere or a slow train make sure you check the travel times. Not sure about shuttle buses between terminals but you can follow signs or arrange for assistance with your airline for a wheelchair in the airport. Some trains take you to Termini Station in Rome 30 mins to travel where you can get a fast train directly from Termini Station to Florence - S.M. Novella. There are several trains every 15 mins each day and I would look on the website Trenitalia.it-inter.com

Italotreno does not serve the airport, but you can take a Trenitalia train to either Roma Termini or Roma Tiburtina Station and check italotreno's schedules and fares once there.
