We will be staying in Ischia for a few days at the end of our RS tour in October. We are hoping to fly from Naples back to Seattle in a day or two- but struggling to find flights that will work. I’m thinking Naples to an East Coast city then hop a flight to the west coast.
Any suggestions?

There are direct flights to EWR (United) and PHL(American)
Both are seasonal so may not be available too late in October
Another possible option is thru Dublin on Aer Lingus

Have you searched on Google Flights?
Where are you flying into to start the tour?
You will want to book open jaw flights-into one city, out of another ( multi-city under Advanced Search-not 2 one way tickets)
I agree, look at google flights, random check shows flights from Naples to Seattle thru numerous airlines including connections thru Heathrow, Dublin and Instanbul.
One stop flights available with connection via Paris, London, Dublin, Frankfurt,, Amsterdam, Chicago, Istanbul, Munich, Philadelphia.
What is causing the "struggling".? Seems to be plenty of choices.
Enter from NAP to SEA on Kayak or Google flights and you will get several options. If you are looking for non stop flights then there will be none.
As an alternative, had you considered the Alaska Air flight ( non-stop) from Rome to Seattle?
Leaving Ischia to spend the night before the flight in Rome or Naples isn't all that much different..
Have you looked at British Airways? We are flying from Naples to SFO on British Airways with a stopover in LHR. Great airline and reasonable prices.
