We would like to stay in the center Florence and Rome. Has anyone stayed in a hotel that has a rooftop terrace where we could have drinks and includes breakfast? We will be going at the beginning of May.

They have rooms with a private terrace. We didn't stay in one since we were there in March. They have rates with and without breakfast.
Location was great and our room was spacious.
In Rome, we stayed at Albergo Cesàri and had an enjoyable stay, it has a roof top bar and breakfast area and is mid-way between the Pantheon and the Trevi Fountain.
In Florence, we stayed at Hotel Silla. It's ideal if you're driving as it is close to everything, is in a night only ZTL (driving permitted during day time) and has parking available.
Hotel Hermitage very close to the Ponte Vechio in Florence. Caters to English speakers and has breakfast on the roof.

We stayed at the Hotel California in Florence. It was very good and only two blocks from the Duomo.

I stayed there in 1989, but Roberto (our poster originally from Florence) indicates it's still nice.
I suggest the Hotel Torre Guelfa in Florence: http://www.hoteltorreguelfa.com/ . I stayed there last summer and loved it. Great location close to the Ponte Vecchio and an easy walk to other sights and the train station, good breakfast, friendly and helpful staff, and the most amazing view from the top of their tower. You can buy a drink up there and enjoy taking in the view :)
Rome:
Albergo Cesari has a rooftop bar but be sure to check when they start putting chairs out, I booked that place in May as well and believe I read they open it for seating later in the month. It is in use as a place to look out but not meals and drinks in early May. At least that is what I recall, Stairs to get up there as well so be aware if stairs are an issue.
The Palazzo Navona near by has a lovely roof top terrace with bar but not breakfast. Stayed one night, nice place just be sure your room isn't by the stairs or elevator as passersby can be heard on the stone floor(no carpet).
Florence: Loved the Hotel Brunelleschi for 8 nights, great location, no roof top terrace but a great hotel and staff. Expensive in season.
Albergo Cesari had an elevator to the roof when I was there in July, 2015.
