I will be walking with a crutch, so I need to know for sure what I need to do to avoid standing too long in a line. I will get the card and understand I will skip the ticket buying lines, for Duomo, Accademia & Uffizi will I still be in line a long time? Do I need to make a reservation also? Would that lessen the wait time? What about the Duomo climb, I see on the Firenze Card site it says that I still need a reservation for that?

I'm just curious: how are you expecting to make the climb to the dome with a crutch? That's going to be MUCH more strenuous than standing in a queue!
Yes, you need to make a reservation to climb the dome. From the Firenze card website:
"You don't need to make any reservations with Firenzecard, except for the Dome (Cupola). For the Dome (Cupola)the booking is required - you have to make it at the ticket office in piazza San Giovanni 7 after the collection of the free ticket. Firenzecard lets you visit the museums when you want to and you enter directly into the museums through the reserved entrances. Just go to the priority access following the “Firenzecard” signs and show your card to the staff of the museums."
You will be admitted as quickly as possible to the Uffizi/Accademia unless they're full to capacity and must hold the line a little bit until some visitors leave. This is more likely to occur at the Uffizi. The Card allows you to skip ticket lines but not security check queues. Why would you pay for a reservation if you're already purchasing the pass?
The Duomo itself is free to all so the card doesn't allow priority access there.
