Final Itinerary!!!please check!

Fly into Milan (1 night)
CT (3 mights)-
Florence (2 nights)-
rent a car for Tuscany (4nights)
dolomites ???or lake garda(3 nights)
venice (3 nights)

how can I get to lake garda from tuscany? should I keep the car from Tuscany to venice?

and where can I fit Verona in this itinerary?

First of all if you arrive in the morning at MXP you may not need to spend a night in Milan. If you fly out of Milan MXP you should spend your last night in Milan (the night before the flight) not the day of arrival. Unless you are not going to fly out of Milan, in which case maybe you want to visit Milan the afternoon of your arrival day.
In Tuscany, once you get a car, make sure you find accommodations outside of Florence. Florence with a car is not a good mix.
To drive from Tuscany to Lake Garda is easy. And yes you should keep the car since you plan to have it at Lake Garda and Dolomites.
To get there , of course I don't know where in Tuscany you stay, but let's assume you stay between Florence and Siena, you would take the freeway Siena-Florence (Raccordo FI-SI). Then at the end of that freeway go around the roundabout (silly modern art diagonal statue in the middle). Go through Toll booth of A1 Autostrada (you are at the Firenze Impruneta entrance). Get ticket at the booth (you'll need it to pay when you exit the freeway system). Follow directions to Bologna (north). Then near Bologna keep an eye to directions to Milano. Near Modena keep an eye to directions to Verona (Autostrada A22). Then I don't know where you want to go at the lake. You want to stop in Verona first? Going to Sirmione first? Going all the way to Riva (North lake)? your itinerary will determine where you want to exit. If you are going to Sirmione and then up the western shore, then at Verona airport you would follow the directions to A4 to Milano and exit at Desenzano/Sirmione. take a look at a map and decide the itinerary. Verona is east of the southern shore of lake Garda. You could hit it first if you want. I would. Unless you need to go back to Milan after Venice. In that case you could stop along the way with the train.
One more thing. when your Dolomites tour is over, drive to Venice and return your car upon arrival. You don't need a car in Venice. The rental car returns are all in Piazzale Roma, across the Liberty bridge in the middle of the lagoon.

I am actually staying in agriturismo in sienna. so I guess I could drive to Verona first and then go to sirmione. I did not know about Sirmione until you mentioned it, but it looks beautiful!! I would totally go there!
And I guess I could go to lake Garda after that.. and how long is the ride from sienna to Verona?
