Viator.com is a tour consolidator and they can sell you tours on smaller local tour companies. I used them in Italy, France, and Estonia. I found it very convenient to see which tours are available in each location 1/2 day, full-day, a few hours.......I found lots of choices.
I was very pleased with all the tours I have booked thru them.
I took six tours with Context Travel in Florence and Rome this summer, and all were very good. I see they do offer tours from cruise terminals in both cities. I don't have experience with those specific tours, but I think Context does a very good job and I like their small tours (max 6 people).
Try toursbylocals.com. We used them last summer on a British Isles cruise.
In addition to any responses you get here, look at Cruise Critic. Here's their Italy Ports Forum: http://tinyurl.com/bwqtmhk
We used RomeinLimo for Florence/Pisa and Rome. We got a guide for the Vatican through them also. We found a group on our Cruise Critic roll call to share with us. Both drivers were excellent, on time, friendly, and informative. The Vatican guide was amazing as well. If we were looking to do something similar again, I wouldn't hesitate to use them.
Look at smartcruisetours.com too.
