We are going to Italy next week. Did anyone who recently traveled to Italy have to fill out the ETIAS form (European Travel information and Authorization System)?

That hasn't been implemented yet. I think the new date is in 2023, but I don't remember the precise date.

Right, it won't be required until mid next year or so. As far as I know, there isn't an official website for application yet but this is a pretty good overview of what it's all about. Just don't confuse it with a visa as it's not the same thing. People who need visas to enter the Schengen, for one reason or another, will still need them.
