EU PLF Forms ??? Italy and France

We will be flying from LAX to Italy...are we still required to complete this form prior to entry into Italy? Does the airport supply the form or do we just do it online ahead of time?
Also, later in our trip we will fly from Italy to Paris, again, do we need this form completed for that trip as well? I was on the app.euplf.eu/#/form website and started completing the form. They had an area for a family member traveling with you, is that all that needs to be filled out for my husband or does he need a completely separate form completed? Am I even on a legit site?

Any help or advice would be appreciated.

Your airlines should inform you of what docs you need
Just passport, basically

The form is no longer needed for Italy. France does still require the ePLF, your airline in the US will not check, not sure if your airline in Italy will. For the sake of caution, I would go ahead and fill it out for France, no one will ever check likely, but in case, you have it.

Each person needs to fill out a form, the space for family is just a cross reference, since if you were exposed to covid, they would be too. Yes, you are using the correct site.
