EU Passenger Locator Form (Help in Filling out)

Hi. The EU Passenger locator form is so confusing. Does it want me to list each leg of our trip on the form? I asked my Swiss friend (they don't use the locator form, but he's helpful), and he said to only put in the leg that LEAVES the US (we have 2 legs in the US), and the leg that Arrives in Europe, (we have a 2nd leg in Europe, Munich to Rome). I cannot seem to confirm that and my head is about to explode, as this form has flight numbers and seat numbers needed, etc.

Anyone familiar with the protocol?

Do Americans traveling solely to Italy have to fill out a passenger locator form? We recently traveled to Greece and as of March 15th, they had done away with that. If one still needs to fill out a PLF for Italy, could you please let me know where to get this. Thanks!

I found it online from BA's what-to-know-before-you-go information site. We completed one for each of us and included only US to LHR to our final destination in Italy and the first hotel. I haven't seen anything from BA about uploading this yet. I'm hoping that by the time we leave in May, it won't be required anymore. It is not a very well-designed website.

Get any kind of help with this that you need before your flight. Because of a flight change, we didn't know we needed it until we got to JFK. Only because of the VERY valiant efforts of a British Air employee were we able to fill it out at all (wasn't easy). Keep at it - I understand the "head about to explode" feeling. Our guy said to leave stuff blank whenever you could get away with it but we did have to find the address of at least one place where we'd be staying. It's endlessly confusing. I hope someone can help you more than I can.

We're flying to London today on United. They don't list this as required any longer.

OP is flying to Italy. Italy is not in UK.

According to these sites it is still required for Italy:

and I just checked my Travel Ready on United and it is still required for Italy

You should be able to find it on your airlines website in the "travel ready" or similar section.

You only need to include the flight that LEAVES USA and the flight that takes you to Italy, along with the FIRST place you are staying. Each traveling adult needs to fill one out.

The purpose of this form is to notify you if you have been exposed anywhere along your way.

Important information
Passengers entering Italy are currently needed to complete a Passenger Locator Form via the EUdPLF application (https://app.euplf.eu/).

ITALY Information for travelers with Italy as destination country: All
passengers wishing to enter Italy, via all means of transportation,
will be required to complete the digital PLF (https://app.euplf.eu),
prior to entering the country. Domestic flights do not need dPLF
Passengers in transit who remain within the Transit Area of the
airport do NOT require to complete the dPLF. If for any reason one
exits the airport, a dPLF will need to be completed. One form must be
completed for each adult passenger; accompanied minors can be
registered on the form of the accompanying adult in the Declaration
section. In the case of unaccompanied minors, the dPLF must be
completed by the parent or legal guardian before departure. Check the

Yesterday, that is 01.april, i rode the train from Germany to Milano. There was someone walking thru the train to check vaccinations status and onther one for plf. The person for the plf only checked if you got it digitally and collected the papers from those who printed it out. But he also got empty formulars for those that got neither.

That said i filled it digitally a day before with only the train from Zürich to Milano and not the train from Stuttgart to Zürich. Not only that i also filled out with the wrong train since it was possible for me to travel earlier than expected.

He only checked with his eyes for the qr code so all was good.

The Italian Ministry of Health website was updated yesterday and says it is required until 4/30/22. Who knows if it will be extended?
