EU Digital Personal Locator Form

Having a problem getting past the section that asks where you are staying. The instructions say that you need only list your first hotel. I do that and it keeps asking for more and more hotels! Can’t finalize the section.
My only thought is maybe it’s because I’m using a newer iPad?

Just fill in what you can……mine did the same thing but I recall that listing my first hotel satisfied them.

I am not supposed to divulge this secret, but, the forms are not scrutinized nor reviewed. As long as you satisfy the workings of the form, complete it, and are able to submit it...you are fine. No one actually looks at it. The airline is supposed to assure you have it, but that ranges from just mentioning it to having you upload a .pdf of the document; at immigration, it is not needed.

The form is only needed in the unlikely event that a positive case were reported on your flight, then the Italian Government can pull up your form and see the location of your first stay. After a couple days in Italy, the form has finished being useful.

I suppose in the worst event and you are contacted about an exposure, if there are serious errors, or flat out deceit, on the form, the authorities could take action.

Question for Paul. Do you think we're okay to put "unknown" for the seat number? The form seems to take it. We aren't going to be assigned seats till we're at the airport.

I can tell you that I filled out the forms to get from Ireland to Italy and it's basically the honor system I suppose unless they spot check some people. Nobody at the airline (Ryanair) or at the customs in Italy actually checked my CDC vaccination card. Perhaps they do sometimes.

Rick, nobody is supposed to check the CDC cards of US nationals flying between EU countries: the airline was supposed to do it in the US, before letting you get on the plane. The check-in staff in the US should do it any time, not only sometimes.

Just like there are no checks on the local trains between the Cinque Terre villages and La Spezia with no reserved seats, whereas your CDC card should be always checked on the long distance trains from Tuscany to La Spezia and Monterosso.

Regarding seat numbers: Technically the instructions for the Italian form indicate to fill in the form between Check-in and departure, I suppose just for the purpose of having the seat number. Those that have seat numbers, have filled in the form weeks ahead with no issues, and I can say that on one flight, they changed our seats...upgraded us, we did not go in and fix the form, and no issues were encountered.

I suppose in the case of changes, the airline has those records, the only real issue would be if they routed through a different Country, and it happened to be a high risk one. As for the seats, I have seen some people have indicated seats in economy that were open on the flight, if the airline seated them elsewhere, then, oh well. Again, their final seating will be recorded by the airline.

One note about the CDC card and if they will check it; technically (caution, it changes all the time) the vaccination card is not a requirement for entry, like the test, so airlines really do not need to check it, they have only officially been delegated the task of checking the test (and only US airlines originating in the US flying direct to Italy or an itinerary with Italy as the final destination).
If you transit somewhere in Europe, they will not check the CDC card unless it is a requirement of that country that you need a green pass to fly...so any internal flight within Italy for example, you will show your CDC card, probably several times.

I confess that I have crossed the border re-entering Italy six times since May; I was asked to show the pdlf only by an Austrian railway officer at Innsbruck station and at the check-in at Dubai airport, but never by Italian officers. While some checks are dutifully enforced, for example the green pass ones, the dplf looks not really enforced, so do not agonize about filling some arcane fields.
