Elba or Ischia?

We are planning a fall trip in Italy and would like to end with 4-6 days to vacation after the vacation on one of the islands not far off shore. Last year we spent 4 delightful days on Ischia but would consider Elba too. Having not been there, we could use and would appreciate some suggestions. We will be taking public transportation and our timing and route are flexible at this point.

Like valadephia, I think it's going to get down to whichever fits best with your itinerary.
I've been trying to fit Elba in my last 3 trips but it does deserve at least 6 days.

4 days in Elba are good, but one should look at your overall itinerary to determine if it make sense to detour there. Is your itinerary taking you to Tuscany at all?

Our itinerary is entirely flexible. Taking in one of the islands will be a highlight and would determine where we go on the rest of the trip. It'll be our 5th trip to Italy and we've visited many of the main tourist centers. We really enjoyed Ischia but wonder about the attractions on Elba. Hiking, eating and a little beach time are key.

I would still let the rest of the plan guide the decision. I would not pay 50% more to fly into Genoa, Florence, or Pisa to visit Elba for example, if I could fly into Naples at a better fare. What is the gap in your mainland travel?
