We have an early flight out of FCO at 6:00 am. We're staying at a B&B near the termini station, but the train apparently doesn't start running until 5:15 am.
What would be options to get us to the airport early enough? We would likely need to leave around 3:00 am to be safe.

I am guessing that reserving a cab would be the best way to go. Maybe your B&B host could help you do this.
Another possibility (if you are willing to spend the money) would be to spend your last night at the Hilton that is attached to the airport. You can take the train there in the evening after you are done with your day’s activities, and then get some sleep before walking 5 minutes into the terminal. I often do this when I have an early flight and did this just last February in Rome.
At 6am it is most likely a domestic flight so getting to the airport around 4.45 to 5am would be adequate. Not many flights at that time of morning so it is only check-in and security. At that time of the morning the taxi ride would be only 30 minutes or so. So a departure around 4.15 would work. If it is easy, walk to Termini and grab a cab.

As suggested, it may be better to take a taxi - Piazza dei Cinquecento (Termini Station)
When the taxi gets to you, you will see the meter is already running. The taxi turns on the meter as soon as it is dispatched.
But, I think it will be a fixed price if you grab one right at Termini in queue - “Comune di Roma.” Double check.
We always stay at Hilton FCO when we have a very early flight. You can walk or take the ahuttle to the terminal.
If you are in Rome already for a few nights before, I don’t think it makes se se to change hotels and move to the Hilton airport just for the last night. Also the Hilton costs at least $250 a night, so it might be significantly more than the B&B you have booked.
At that time in the morning, your flight is certainly a intra-Europe flight. Try to leave your B&B at about 4-4:15 am. The taxi should get you to the airport by 4:45, which is more than sufficient for your flight..
What Airline are you flying? Two years ago we had a Turkish Airlines flight to Istanbul that departed FCO at 6:00 AM. We stayed at the Hilton the night before, just to make things easy for us.

We'll be flying on Lufthansa to Frankfurt from Rome. (On our way back to US/Denver). Is that easier because it's intra-Europe to start? (i.e. Is it less important to get to the airport early?)
Germany and Italy are both in the EU in the Schengen zone, therefore it’s like flying a domestic flight from Denver to San Francisco. You need to have completed the check in procedures and have a boarding pass in your hands at least 45 min prior to departure. That is when they stop accepting luggage at the check in. Being in line at the check in counter at least one hour prior to departure should be sufficient. Although I like to make my grand entrance at the airport at least 75 min prior. That gives me half hour to deposit my bags at the check in, way more than sufficient to have time to spare for a cappuccino and brioche at the bar before boarding.
