We will be arriving on Monday, Nov. 7,2022 by cruise ship. We r scheduled in port from 6am till 6pm. We r interested in hiring a driver/tour guide from the port to Volterra and also San Gimignano. We r open to other suggestions but have already visited Florence and Lucca.
Also, is it worth visiting Ischia from cruise terminal in Naples at the beginning of November

This may be a stretch but the RS recommended Turislucca runs a number of Livorno cruise based excursions:

They don't offer one for Volterra but they offer several others - Lucca, Pisa, Florence, wine country etc - and they might willing to modify a tour to your requests.
And if not maybe they can recommend someone who can?
FWIW I took one of their Lucca walking tours and really enjoyed it.
