Could anyone recommend a reputable driver to pick me up at the Milan/MXP airport and drive me to Vernazza and then a return trip back to Milan one week later? Need contact information and pricing.

That is going to be one expensive trip. Have you thought about catching the train. Around 3.5 hours from Milano Centrale.

That's going to be at least $1000. Probably more than your airfare to Italy.
By train it's much less and just as fast. Take a Malpensa Express train from the airport to Milano Central. Then another train from Milano Centrale to Monterosso. Vernazza is the next village, but you'd need to change trains in Monterosso as only regional commuter trains stop in Vernazza.

If you want to go by car at all costs Google:
"Noleggio con conducente Milano"
It means car hire with driver Milan. Lots of companies will pop up. Inquire prices via email but prepare to pay well over $1000 round trip.
