A rental car costs more when you drop it in a different country, unless you are later returning it to Switzerland. By train, you can get from Lausanne to Genoa in 5.5 hours, some with only one connection.
Is your destination the big city of Genoa, or some of the smaller towns of the Italian Riviera?
There are quite a few places you could stop in Switzerland or Italy. Can you provide more info about the length of the trip and what you're looking for?

As Laura mentioned, there's often a substantial charge if you rent a car in one country and drop in another. Also note that each driver will require the compulsory International Driver's Permit, which is used in conjunction with your home D.L. You can easily obtain an I.D.P. at any AAA office for a small fee.
Again as mentioned, if you'll be visiting the Cinque Terre, a car will be of little value since no cars are allowed in the five towns.
