I’ve been hearing conflicting reports that I may or may not be required to have one. My friend just got back from Greece where he was barred from driving and he says I should get one. Everyone else says that as long as my California license has Roman letters, I’m ok.
Everyone else is an idiot. Go to AAA, spend $25, get one and carry it to Sardinia with you. Easy peasy.
If your licence was issued by a non-EU/EEA country (for example the USA), then you do need an International Driving Permit as well as your original licence in order to drive in Italy (of which Sardinia is part).
This is not a requirement of the rental company, it is a legal requirement to drive in Italy. Without it you are driving illegally, and if stopped by the police, or you have an accident, you would have expensive legal problems.

I agree with the first reply. "Everyone else" are ignorant of the rules and should be ignored.
An IDP is compulsory for driving in Italy, which includes Sardinia, and you can be fined on the spot for failing to produce one if requested. These are used in conjunction with your home DL so you must take that also. IDP's are valid for one year.
The main function of an IDP is to translate the terms of your home DL. "Roman letters" are not the issue. The terms of the license have to be presented in a format that can be understood by local police.
The AAA issuing office can also take the photos for you.
An IDP and an eye for sheep on the roadway will go a long way in Sardinia.
