Customized small group 1-day tour Rome

I am writing on behalf of a small group (4 to 8) that will be in Rome for 1-day on a cruise stop. We arrive at 8:00 am and depart at 8:00 pm. All of us have visited Rome previously, and don't want to revisit many of the typical tour stops (St. Peters, the Vatican-including museum, Parthenon, Spanish Steps and Trevi Fountain). Perhaps, unusual, but we have not visited the "ancient city" Forum, et al. and Coloseum) and we've not visited the Baths of Caracalla, Baths of Diocletian, Catacombs of Priscilla or the Villa Borghese (including the Borghese Gallery). If time permits we like to stop at the Victor Emmanuel Monument for an elevator to take-in the aerial view of the city.

We are hoping to find a guide who can provide transportation between Civitavecchia and Rome as well as point out specific nuances we might miss if we were visiting these venues on our own. A guide would also be able to better organize the order in which to visit these places to avoid back-tracking, long lines and limit walking long distances. By "long" I mean keeping walks at any one stop to 1,500 meters or so. The name, email or web address of such a tour guide would be greatly appreciated.
