My Cpap machine automatically converts to the voltage for Italy. There's no switch to convert. I know I'll need to take an extension cord because of the limited plugs in rooms but my question is do I take a 110 voltage cord or do I buy a 220 one? I'll use the correct adapter.
If anyone can help I'd appreciate it

My hubby just brought his to Italy & France. He brought a regular 110 extension cord and used the correct adapter per country - no issues.

You're not the first to ask this question: https://community.ricksteves.com/travel-forum/general-europe/c-pap-help. There's a good answer by Nigel toward the bottom of the thread.
I don't even bother with extension cords. On all my travels to Europe (20+) I've only ever needed one in a Venice hotel on one occasion.
Beg to differ. In older hotels/B&B's/apartments, usually the plugs are situated in inopportune places, and there aren't many of them. You want the CPAP right next to the bed. We always bring a dual voltage power strip, and an extension cord besides just in case. I would say half the time we've used the extension cord. And we follow the credo 'pack light, pack right'.
In case you're interested, Respironics has a smaller CPAP now than their previous one. My husband just switched over before our trip and was glad to have the smaller footprint in his luggage.
