You can purchase your ticket, add a tour and add the Underground and/or the Belvedere.
The official guides are excellent and group size is limited.
We enjoyed the tour through Co-Op Culture. It was a challenge getting the underground/area/belvedere tickets, but worth it. Very reasonably priced and really interesting tour.

Save some money and just book the entrance tickets. Download Rick's Audio tours of the Colosseum and Forum. I've used both a tour company on a trip (Walks of Italy), another trip the Colossum guides they offer, and another Rick's app. Every guide will tell you something different, focus on items like Architecture over History, and vice versa. Each one we've heard different items. take a listen to Rick's app ahead of time, if that works for you then you are at your leisure to wander around and not have a guide rush you or you lose them momentarily trying to take some pictures.
Also both there and in the Vatican there are a lot of people and guides that from time to time our ear pieces kept hearing a different tour guide if we took picks and the guide walked further ahead. Rick's app was perfect to just pause anytime we wanted.
