Colosseum ticketing question - guided tour includes what??

I have tried to read between the lines on the official ticketing website but I remain confused: do the guided tours of the arena and/or underground ALSO include a guide with the upper "standard" levels of the Colosseum or just the arena area? And does it matter?

We used Carpe Diem Tours in March. The guide was very knowledgeable and we were pleased with the small group tour. With the tour we were able to skip the line. Be prepared to show your passports.
