If you purchase a tour of the Underground you also have to purchase a general entry ticket that covers the Colosseum, Forum and Palatine. They can be purchased in advance on the website or at the desk when you arrive for your tour. The Underground tours fill very quickly so you'd want to make your reservations right away when slots become available for the day you want to do it:
"It will be possibile to book on website underground Tours for individuals and underground group entrances every third Monday of the month for the following month".

Yes I would recommend a tour. We used Walks of Italy which allowed us to skip the line and covered The Colosseum, Palatine Hill and the Forum and the Underground and another tier that did not allow the general public (people tried to follow us up once the gate was opened) and when you buy the tour it includes your entry ticket.
We took the Underground tour and loved it. My 7 year old loved it.
The colosseum is very well signed so a tour isn't necessary. There are many free audio guides you can get if you want some extra information.
The underground tour is extremely popular so if you wish to see it you should book as soon as it becomes available. Just a note, you can only purchase 6 underground tours per email address and per credit card. If you try to purchase more the reservation is automatically canceled. Be sure to have two different people make bookings or you will risk not getting any.
