We'll be on a cruise in Aug. We have around 12 hours in the port of Livorno. Can we see both Pisa and some of Cinque Terre ? What if we get a car or car and driver?

IMO, you'll only have time to see either Pisa or the Cinque Terre. You'll have to allow for transportation times from Livorno, regardless of whether you use public transport or hire a car & driver. Of the two, I'd recommend visiting the C.T.
I'm sure the cruise line will be offering overpriced tours to both areas. You might check local bookstores to see if you can find a copy of the RS Mediterranean Cruise Ports Guidebook, as I'm sure there will be lots of good information there on the various day tour options from Livorno.
