My husband and I will be traveling to Sicily in October and want to drive up to Cianciana, from Agrigento to see that town (about 44km)Is there any safety reason to avoid leaving the main roads of sicily?

I've not been there specifically, but have been in rural western Sicily and felt perfectly safe. While certainly not as wealthy as northern Italy, it's not as if Sicily is a third world country. Enjoy!

No danger at all. The SS118 is a good road and you will arrive in Cianciana in a little over an hour from Agrigento.
