Last month the TIM store had a phone for 39 euro including SIM card and five euro call time. No hidden charges. TIM stores are in all the major towns and most smaller ones. Also check Vodaphone and Wind stores.
If you subscribe to Verizon, you can rent a phone through Vodaphone. I use this service and forward my office and US cell phone direct to Italy. Might be a little spendy, but the service and coverage has been fantastic.
you can buy or rent a phone from cellular abroad. I do not think they are as cheap as listed above.

Again, check out Mobal.com. You only pay for the phone ($49 for Europe) - and for each call ($1.25 + per minute - depending on the country - forever (no rental fees). We've had ours for about 5 years. Works fine.
I have TIM service for Italy and have found it very reliable and the personnel in their offices very helpful. I purchased the phone on E-Bay and bought a SIM card and minutes when I got to the Termini in Rome. If I had it to do over again, I'd purchase the phone in Italy (as Henry suggests above). You can have minutes added at many "tobacco" shops throughout the country.
