There’s not many cons. Outside of the couple main cities a car is kind of a necessity to effectively see the island. You can limit the rental a bit by taking public transit from the airport to any nearby city, that’s one aspect the public transport system covers well. Driving is pretty easy overall, the autostrada is quite good. Use it where you can.

Every rental company has its share of horror stories. I’ve rented from Europcar and not had any issues.

We used Hertz and were very pleased. Would recommend renting the smallest vehicle that fits your needs as parking, and possibly a street or two, could be quite tight.

We used Avis about 6 years ago. Picked it up when leaving Palermo (office was close to our hotel so just walked there with our bags) and returned it some days later at Catania airport. No issues and no hassles.
