Is there a reason you would not just take the 2 hour train into Florence?

The easiest locations we have found in Florence for car pick up and drop off are on Borgo Ognosanti which has easy enough access to the Autostrada. We have done this location several times.

Hertz is the only agency in Orvieto

You can check with autoeurope.com but they had dropped that agency due to bad service

Assuming you will have a designated driver if you are wine tasting? Or are you staying over somewhere in Tuscany?

You can use the Search box top center to learn about insurance in Italy, ZTL restrictions, and IDP needs. Of course the usual car rental issues of drop-off fees and volatile pricing post-Pandemic apply. Have you ever rented in Europe before? You might get a diesel or a standard transmission, no matter what you order. You need to check for required safety equipment (I don't remember what it is for Italy), either because the returning agency will look for it, or because a police officer might look for it.

If you’ve never driven in Italy, you will need an IDP ( international drivers permit). Available at AAA or online. Cost is about $25.

For italys driving laws, download this most comprehensive book. Yellow banner at bottom of page.

Rather than Orvieto, train to Chiusi-Chianciano terme and pick up your car there. Train is 2:11 minutes. This puts you about 30 minutes from Montepulciano.

Edited for spelling.

In addition to the IDP, drivers in Italy will also have to contend with the ZTL (limited traffic) zones that exist in many towns in Italy. Many of these are enforced by automated cameras, and each pass through one will result in hefty fines, which you probably won't know about until several months after you return home. Florence is extensively covered by ZTL cameras so you'll have to be careful where you drop the car off.

There are also tolls, fines for driving in bus lanes and speed cameras including the Traffic Tutor which measures not only instantaneous speed but also average between two points.

If you are coming from Rome, I would rent in Rome and save myself the hassle of taking the train to Orvieto (Hertz) or Chiusi-Chianciano (Europcar, Avis-Budget).

Florence drop off is on Borgo Ognissanti at the Interparking Garage Europa carpark for all car companies.

I posted the instructions on how to reach it in the post below (see 3rd comment in the thread):

Hertz also has a location on Via Ambrosoli, near the Firenze Sud Exit on the A1. From there you will need a taxi to the city center.

Florence airport car rental center is very inconvenient for those coming from the south.
