Does anyone have recommendation on car rental company in Ischia? Their websites are all in Italian - difficult to figure things out.

Most places rent scooters and small cars like smart cars. They are all local companies. For the major companies you need to go to Naples.
If you go in summer be aware of the traffic congestion and the lack of parking. That is a place where renting a scooter or a moped is a good idea (assuming you know how to operate one). There are also buses that go around the islands as well taxi cabs. Also don't forget the Sea Taxi boats. Those are great too and are cheaper than the ones on wheels.

We used the buses on Ischia and it was easy.
