Car rental from Milan

I found some cars at around 30 US dollars on sixt per day. We are picking and delivering it in Milan. We are planning on going to Lake Maggiore or maybe Lake Garda.
My questions are, is it worth it? Because it seems easier to travel through the towns around the lake if we have a car. Do they have reliable transportation around either lake?
Another concern is that, for example, the results I found for the cheapest car is 132 US dollar for the 3 days. If I book it from the website, am I going to get charged more when I pick it up? Driver age is 23.
How much is the gasoline? How much do you think we'll spend in gasoline in those 3 days? Do you think that is the tank is full we can manage for the 3 days with that to get to the lake and drive around towns?
I guess that I have to return the car with the tank full, how much would that cost if we fill it up in Milan? The car says to be a Sedan | MCMR Smart Car for 2.

I (and many contributors in this forum) use consolidators for renting cars, such as:
www.autoeurope.com
www.kemwel.com
(both are part of the same corporate umbrella after a merger years ago, but check prices in both sites, as they differ).
They work with the major rental companies, primarily Europcar, Hertz, Avis-Budget, but they get better deals than directly.
Give them a try.

Regarding your question.
I've visited lake Maggiore and lake Garda more than once, but always with a car, except for one occasion (in the 1980's, when I was a poor university student), when I used the train (and buses and boats) for Lake Maggiore and Lake Orta.

There is public transportation in all lakes. Ferries, private boat excursions, buses, and to some extent, trains.

It is entirely possible to do without a car and rely on the above described transportation options. I prefer the car, because it is often faster and more efficient, also because buses and ferries don't run much at night, therefore you need to be aware of schedules if you go someplace else for dinner.
