If you don’t have a phone with an Italian SIM card the best option is to get a Google Voice Number before you leave. I use Google Voice to make calls to Italian landlines (including while in Italy). You can use Google Voice overseas, but if you don’t have a Google Voice number already, you must sign up and open an account with a Google Voice number while in the US. The cost of calls to Italian numbers using Google Voice is one cent per minute. If you make a regular call from your cell phone without using Google Voice, your cell phone provider will charge you for that call as an international call to Italy, and it is not cheap no matter which provider you have.

I have used it about 12 years ago. We knew where we exactly going, so while it was a bit slow, it was very doable. We had looked at maps prior to traveling to Italy so it was very easy. Our friends who did not know where they were going, had a much different experience; they had a very stressful time.
This was prior to usage of navigation apps and we used maps, so i assume it is much easier now.
