I use AutoEurope (a broker) to rent a car in Italy. They have 3 websites. Check all 3 as deals vary.

Last time I was in Sicily, the car rental I picked was Sicily-by-Car, an Italian company based in Sicily.
There are virtually no tolls to pay on Sicilian freeways. The only one with tolls is the A20 - Messina - Palermo which has tolls between Messina and the junction with the A19 (near Termini Imerese).
If you happen to take the A20, just pick up the ticket at the toll booth when you enter at the freeway ramp, then when you exit take the gate with the attendant where you pay cash, give the ticket to the attendant, look at the price on the display, hand the cash, and proceed.
In the image below is the toll gates when you exit. The cash gates with attendant are the ones on the far right with a white sign displaying with the image of a black hand and the cash (they accept no cards at the attended toll gates). The ones with the cash image but no black hand are cash machines where you insert the cash inside yourself (they give change, so no need to have the exact toll amount). The latter accept cards as well.
I've used SRC Rent Car website. No problems at all, good car and hassle free.

We also rented a Sicily by Car car through Autoeurope.com. They were great and gave us an upgraded car asa surprise.
