Will be renting a car at the Rome airport. Want a small car for 4-5 adults? any suggestions? Will only have carry on luggage from airport to villa and then do not need luggage space.

I'd look into a Compact SUV or an Intermediate size for that number of people. Small cars (Economy) are the size of a Fiat500X, and that is tight for 5.
Don't choose Mini size, because those are basically two seaters, like a Fiat500 regular, or a Smart

When we went to Germany and France we rented an intermediate car and it was a Hyundai Tucson. For our Spain and Portugal trip we rented a compact SUV and it was a Seat Arona. With five adults, even with carryon only, the intermediate car would be too small. You probably could get by with the compact SUV, but even it might be tight regarding luggage. Our rental company was Enterprise.
