We, a 70 yo couple, are torn between the 3 or 4 hour tour. They make all the same stops, with the difference being you might have more time for the Blue Grotto and I assume more swimming time with the 4 hour.. We will probably go in the water, and being from the Northeast, we can handle cooler water temps. I am just thinking about sitting on the boat wet , while we make the other stops.
I know someone who did the 3 hour. She loved it but said they suggested not going to the Blue Grotto because of the wait time. I also understand there is the chance of not getting in to the Blue Grotto even with a 4 hour tour.
I am concerned 3 hours might seem too short and 4 hours too long.
I'd appreciate thoughts on this, especially if you had done one of these.

If you really want the Blue Grotto take the 4 hour tour
The Blue Grotto is a real time suck and can’t predict that or even if it will be open

We did a 3 hour tour last month (cue theme from Gilligan's Island) and it was plenty of time, including 2 swim stops. I am sure 3 hours is enough, especially if it is a private tour. We are 65 yo, and we booked our tour as we got off the funicular and a woman asked if we wanted to tour the island and we said yes! There were 5 people plus our captain, they provided towels, and it was 70 euros each. Our stops weren't really stops, but pauses as you cruise around the island, except for the swimming stops. My primary reason for going to Capri was to go to the blue grotto, but, it was closed. The week before an American couple had to be rescued from inside. After seeing that video and seeing the entrance from our tour, and hearing from our captain how much pressure there is to tip your rowboat driver, I am fine with missing it, and not ever trying again. The cost of the blue grotto is not usually included in the tour price, and it costs 20 euros per person. Have a wonderful time!!
